AMS of Wisconsin, LLC Oshkosh welcomes visiting or transitioning clients into the area and wants to ensure that you keep on your path to recovery in the form of our affordable and convenient guest dosing services.

Cost of Treatment Services:

One-Time Registration Fee: Call for details

There is a daily dosing fee of $25.00 for all guest patients and a one-time administrative fee of $25.00.

AMS of Wisconsin, LLC Oshkosh accepts cash, Visa, MasterCard, and Discover, as well as money orders.

Please note that personal checks are not accepted.

AMS Wisconsin – Oshkosh Welcomes Patients Visiting the Area and in Need of Guest Dosing Services.

Visiting patients will find our professional and patient-centered facilities easy to access, and we strive to provide excellent patient care.

In preparation for guest dosing, each patient’s home clinic will need to contact us via phone / fax prior to arrival at AMS of Wisconsin – Oshkosh.

Each patient’s home clinic MUST fax a signed copy of a guest dosing request form including the signature of the Medical Director.

A minimum of 24 hours-notice must be given from home clinic. If guest dosing is to start on Saturday, all information should be received by noon Friday.

A metal lock-box is required for Sunday, holidays, or to pick up regular take home medication.

Any take home medication must be specified by your home clinic.

To schedule intake, call us today or complete Get help now section and one of our staff will contact you. Please bring a valid photo ID, insurance card, list of medications you are taking, and intake and treatment fees to your intake appointment.

Typically, after the intake day, just a few minutes. We have multiple dosing windows and work very hard to have little to no line.

You must meet with your counselor to complete a Treatment Team Request form to be considered for take-home privileges. Decisions regarding take-home doses are made by the Medical Director in consultation with members of the clinical team. Your counselor can provide you with a Take Home Criteria Checklist.

Yes! In order to transfer to AMS of Oshkosh, LLC, you or your current clinic must provide documentation for the following in advance: discharge/transfer paperwork. These documents must be reviewed by the intake team before you can begin dosing at AMS of Oshkosh, LLC, along with being seen by the Clinic Physician, Nurse, and Counselor.

If you are going to be out of town and unable to attend the clinic for medication, you must inform your counselor as soon as possible so your counselor can make an effort to arrange dosing at a clinic. Each clinic has different requirements.

I am a patient here and never once herd of anyone waiting to get in they do intake twice a week witch is open walk in you don't need a call back just go in open from 530 to 11. Best clinic around counclers are often complained about because they expect people to follow rules it's treatment people life on life's terms now treatment on your terms and then complain about it when it doesn't work out anyways I would recommend it to anybody facing any kind of addiction I would even recommend it to my own daughter if she was facing what I'm going through

the staff is amazing! not a huge fan of the patients, they tend to gossip. a lot of them don't follow the rules which makes the place look bad..like smoking on the property, not showing up to groups, complaining, ect. but its worth it because everyone that works there is great and I feel like my councilor is not only a great doctor, but a great friend. they actually care!
